Substance use evaluations for Tennessee healthcare practitioners referred after a confirmed positive drug test, refusal to test, or a concern raised by an employer, professional licensing board, or peer assistance program.
Work directly with Brian Golden, MA, LADAC II, SAP. Evaluations use the Addiction Severity Index (ASI) and ASAM criteria to inform individualized clinical recommendations. Brian also evaluates DOT-regulated workers in safety-sensitive positions subject to FMCSA requirements.
In-person appointments in Paris, Tennessee, or virtual appointments when accepted by the referring organization.

Tennessee law provides reporting requirements for covered healthcare employers when a practitioner has a confirmed positive drug test or refuses testing, subject to the applicable statutory conditions and exceptions. Reports involve the Tennessee Department of Health and the practitioner’s licensing or certifying board.
Your board or professional assistance program may require an evaluation, treatment, testing, or monitoring. Requirements depend on your profession and circumstances. A referral does not necessarily mean your license has been suspended.
Brian provides substance use evaluations using ASI and ASAM criteria, with clinical findings and individualized recommendations to support the referring organization’s review. Confirm evaluator acceptance and the required assessment scope before scheduling. The evaluation itself does not reinstate a license or authorize a return to clinical practice.

Complete your clinical evaluation
Discuss your substance use history, current circumstances, and relevant workplace concerns. ASI and ASAM criteria help guide clinical findings and recommendations.
Submit findings and follow your referral requirements
Brian prepares evaluation documentation and communicates with the referring organization with your authorization. Any requested follow-up is reviewed against your referral requirements. The program, employer, or licensing authority determines acceptance and return-to-practice requirements.

This is a substance use evaluation. Acceptance depends on the referring organization’s requirements. Additional medical, psychiatric, psychological, or specialized fitness-for-duty assessments may require another provider. Treatment, laboratory testing, and program monitoring are separate services.
